It can be hard to deal with the fact that you will have to leave your home, and perhaps give up some of your independence, in order to get the help and services that you need. Choose Location

The first thing that you will want to do when trying to qualify for assisted living in Minnesota is to determine where you would like to live. You will want to take the time to check out many different places to make sure that they fit for you and will be able to take care of all of your needs. You might be stuck in this facility for a long time so it is important that you feel comfortable there and that you like the staff.

Complete Physical Assessment

Once you have decided on a facility that you like it is time to complete the physical assessment. You will not qualify for assisted living in Minnesota if you do not fill out this assessment. You will need to fill out all of the information that you can about your physical and medical history so that the facility knows what to expect when you come and stay with them. These forms will need to be turned in at least 30 days before you plan to move in.

Sign Agreement

The next step will be to sign the resident agreement, after the physical assessment has been processed. You and your caregiver, if you have one, will both need to sign the agreement for assisted living. The agreement is mostly just a contract stating what services the facility will perform and how much you will have to pay for those services.

Functional Assessments

You will then have to fill out a functional assessment after you have signed your agreement with the facility. The functional assessment will be conducted within the first 30 days that you are at the facility. The staff will watch you and see how well you are able to perform basic daily functions such as bathing, getting dressed and eating. They will then use this assessment to see what kind of assistance you will require while you are under their care at the assisted living facility.

Complete Service Plan

The next step is to complete a service plan, which is always done after the functional assessments have been determined. This plan will list out all of the services that the facility will provide you along with how often and when those services will be provided.
